Position Description

You will be responsible for managing a caseload of approximately 10-15 clients. Provide Independent Living services to adults and youth with disabilities, one-to-one. Candidate must have the ability to conduct needs assessments, develop independent living plans, and provide Independent Living Training.

Duties/responsibilities include but are not limited to:

  • Assist Client in identifying and accomplishing goals by providing skills based training, phone calls, paperwork, resources, and community activities
  • Assist Clients with independence promoting skills, as well as advocating for their needs by teaching activities in areas such as safety, housing, cleaning, cooking, organization, assertiveness, financial management, benefits management and self-care, communication skills, interpersonal skills, self-care, community integration and mobility.
  • Create linkages to existing services and establishing ongoing coordination of these services
  • Locate and access community resources
  • Assist with financial planning for limited income
  • Act as an advocate for the Client
  • Participate in Client team meetings
  • Complete required documentation
  • Work with individuals with a variety of disabilities including elderly, developmentally disabled and mental health.
  • Complete all required paperwork/documentation under 245D and 245A
  • Travel throughout several counties to meet Clients in the community and/or their home.

Required Skills

  • Two year degree in a human/social services field or five year of experience in a disability related position.
  • Must be at least 18 years old, have experience working with people with disabilities and disability issues.
  • Must have Drivers License and reliable vehicle with active automotive insurance

Additional Skills

  • Knowledge of social service programs and operations.
  • Knowledge of laws and ethics pertaining to data privacy.
  • Knowledge of socio-economic factors that promote stable family life.
  • Ability to utilize effective interviewing techniques.
  • Ability to analyze complex situations and evaluate potential for improvement.
  • Ability to organize and prioritize a high volume workload in an efficient manner.
  • Ability to work independently and follow directions and guidelines.
  • Ability to seek out resources and information.
  • Ability to establish and maintain accurate records.
  • Ability to effectively communicate verbally and in writing.
  • Being able to work with a diverse population, low income and individuals with mental illness and physical disability.
  • Work as a team with staff of diverse background and cultures.
