Transitional services

Transition services include items and expenses that are essential and reasonable for an individual to move from an eligible setting to their own home or an integrated community support environment. An individual is restricted to one transition every three years from their last move or use of transitional services, and they can receive up to $3,000 per transition, distributed as follows:

  • The cost of essential furniture may not exceed $1,000 of the $3,000.
  • The cost of household supplies may not exceed $300 of the $3,000.
  • The cost of moving expenses including security deposit and application fee, allotted $1,700 of the $3,000. This budget can exceed $1,700 by using funds from the other two budgets, but the total budget cannot exceed $3,000.
