Our goal is to provide the very best in helping individuals residing in eligible institutions (Hospital, Nursing Homes, Rehab Centers, etc.) to gain access to needed medical, social, educational, housing, other services and support necessary to move from these institutions into a community-based setting.

Position Description

  • Assist clients in locating suitable housing in the community.
  • Assist client in locating community resources.
  • Performs outreach to nursing facilities and other appropriate agencies regarding relocation services.
  • Works with lead agency to arrange details of waiver services.
  • Meets regularly with Clients to discuss progress, problems, and plans.
  • Take clients to potential sites for tours.
  • Assist clients in completing housing application.
  • Coordinates meeting(s), medical follow up appointments, delivery of medical equipment, availability of medications, etc.
  • Develops an individualized person-centered transition plan.
  • Complete CAF to apply for Emergency Assistance, cash, food support, 1st month’s rent deposit, utility bills, etc.
  • Explore use of transitional funding or Bridging referral if needed.

Required Skills

Bachelor’s degree in Social Work or Human Services or related field with experience in Case Management or a two year degree in Human Services with at least 2 years of paid work experience.

Additional skills

  • Knowledge of social service programs and operations.
  • Knowledge of laws and ethics pertaining to data privacy.
  • Knowledge of socio-economic factors that promote stable family life.
  • Ability to utilize effective interviewing techniques.
  • Ability to analyze complex situations and evaluate potential for improvement.
  • Ability to organize and prioritize a high volume workload in an efficient manner.
  • Ability to work independently and follow directions and guidelines.
  • Ability to seek out resources and information.
  • Ability to establish and maintain accurate records.
  • Ability to effectively communicate verbally and in writing.
  • Ability to lift 25-50 lbs.
  • Being able to work with a diverse population, low income and individuals with mental illness and physical disability.
  • Work as a team with staff of diverse background and cultures.
  • Willing to travel long distances to meet clients and tour potential relocation sites.
