Comprehensive Community Support Services (CCSS)
CCSS services are designed to create a nurturing environment where every individual feels seen, heard, and supported on their journey towards stable and secure housing.
To ensure the seamless continuation of care, OCS extends its commitment through the provision of Comprehensive Community Support Services post-relocation. This short-term (12 months) support service is specifically designed to address various aspects:
- Facilitating connections for MHM participants to establish and integrate informal supports within the community.
- Providing ongoing support to maintain housing stability, including tenancy support encompassing dispute resolution with landlords and neighbors, household management, and budgeting assistance.
- Assisting with organizing schedules and appointments to foster a structured living environment.
- Supporting MHM participants in maintaining a continuous connection to community resources.
Authorization
This service is authorized by a waiver case manager with maximum unit of 1200.
It is important to note that this service is offered in addition to waiver services, should a participant be eligible for such support. Our holistic approach aims not only to address housing needs but also to empower individuals for sustained well-being and community integration.
