HSS helps people find permanent housing. People in different situations are eligible. The program could be a good fit for you if you’re already living somewhere that just isn’t working for you, if you’re currently homeless, or if you’re living with a family member or you are in an institution like hospital, Nursing Home, or intermediate care facility (ICF). It also involves creating a housing focused person-centered plan based on your needs, preferences, and informed choice.

To get started, we will need to do some planning. We will do an assessment. Then, we will help you find housing, assist with the move, and do a follow-up to see how things are going.

Housing Stabilization Services

Housing Stabilization Services is a Medical Assistance benefit to help people with disabilities, seniors and individuals that are at risk of homelessness, find and keep housing. Housing stabilization services are state plan (not waiver) Home and Community-Based Services (HCBS).

HSS is composed of three different services: Housing Consultation, Housing Transition and Sustaining

  • Housing Consultation – service to develop a person-centered plan for people without MA case management services. This service supports an individual in building a housing focused person-centered plan. Housing Consultation services also support individuals in making informed choice of a housing transition or sustaining provider, in addition, consultation services assist individuals in connecting to resources that are non-housing related but are identified in an individual’s person-centered plan.
  • Housing Transition – service that supports a person to find housing.
  • Housing Sustaining – service that supports a person to maintain housing.

Eligibility

A person is eligible for housing stabilization services if they meet all of the following needs-based criteria:

Be on Medical Assistance (MA)

Be 18 years old or older.

Have a documented disability or disabling condition.

Be experiencing housing instability.

To get started, a Housing Consultant Specialist will meet with you to complete a housing focused person-centered plan. This plan will help us in providing you the support that you need to find and keep your housing.

How we get it done!

  • Contact the appropriate county agency.
  • Initiate the housing process with potential relocation venues.
  • Look for good affordable apartments for you.
  • Make arrangements for tours to prospective relocation sites.
  • Help you with the housing application process even if you have poor rental history.
  • Assist you in contacting entities with movers, household items, furniture etc.
  • Make arrangements for your transportation on your moving day.
  • Connect you with other community resources and services after we move you into your new home.
